Two things don't count: your first change in your first 30 days as a member, and switching back (or fixing a typo) within 7 days of a change. The limit keeps names stable, so the people who follow you always know who you are.
Yes, if it's been less than 7 days. Go back to Settings → Edit Profile and type your old username, or the name you meant to pick. It won't use up one of your changes, and if you switch back to your old name, everything is put back exactly as it was. After 7 days, a change back counts as a normal change.
No. When you change your username, your old one stays yours. Nobody else can sign up with it or switch to it, links that use it still lead to your profile and poems, and you can go back to it later (that counts as a change). Your profile lists your past usernames under Previously Known As, so people who knew your old name can still find you.
